What to Know Before Your Appointment

New Patient Process

  • All prospective patients complete a pre-screen questionnaire
  • We contact you within 24–48 business hours
  • We will schedule a consultation or provide referrals as appropriate
  • 50% deposit required for new self-pay patients

Telehealth Requirements

  • Patient must be physically in FL, GA, or OH during virtual visits
  • Stable patients on controlled substances require quarterly visits
  • Annual in-person visit required for all patients prescribed a controlled substance. (Frequency subject to change based on FL law)
  • New FL residents must have in-office visit before controlled substance Rx

Attendance & Cancellation

  • Cancel at least 48 hours in advance (excluding holidays) to avoid cancellation fees as stipulated in our policies
  • 10-minute grace period for late arrivals
  • 3+ no-shows or late cancellations in 12 months may result in discharge
  • One-time waiver offered for a single missed follow-up

Communication

  • Patient portal preferred for HIPAA-compliant messaging
  • Allow up to 48 business hours for portal responses
  • Portal is not for emergencies — call 911 or 988 for crises

Insurance, Payment & Billing

  • Paramount Wellness Collaborative is in network with TRICARE East and FL Blue, Aetna, Ascension, Optum/United Healthcare, Quest Behavioral Health, and Cigna through Headway. We are also able to accept insurance through Grow for patients in FL, GA, and OH. For all other insurances, we remain out of network and will provide superbills.
  • Accepted payments include all major credit/debit and HSA/FSA cards.
  • A valid card is required to be kept on file at the time of booking a new patient appointment and to continue treatment. Payment details are not retained by our practice — they are securely and confidentially vaulted through Paramount Wellness Collaborative’s integrated payment platform on the patient portal.
  • A 50% deposit is required at the time of booking all new Self-Pay patient appointments; this is credited toward the total appointment fee, due at the time of appointment. Follow-up appointments will be charged at the beginning of the appointment. Patients paying with insurance will be billed through Grow or Headway.
  • Due to federal and state law, Dr. Ampiah is unable to see patients with Medicare/Medicaid at this practice, even if the patient wishes to pay out of pocket.
  • Upon request, an out-of-network ‘Superbill’ will be available after the appointment, which can be used to request reimbursement from your insurance provider.
